NUTRITION SERVICES + RATES

 

 I strive to keep things simple and affordable for all clients, regardless of insurance or financial difficulties. That’s why I offer self-pay, cash rates with no qualification requirements. Please see Individual Counseling Packages below for self-pay cash rates. I have limited financial hardship options available and income verification is required. Please contact me for specific information regarding financial hardship.

Paperwork, including health history, must be provided 24 hours before the initial visit to ensure a productive session. Upon request, I will provide a superbill.

Blue Cross Blue Shield, Cigna, and UnitedHealthcare insurance is accepted at this time. City of Fort Collins employees can be reimbursed for the visit and I complete the forms.

BEFORE YOUR VISIT, PLEASE CALL YOUR INSURANCE COMPANY AND ASK:

 

  • Is nutrition counseling covered? (the procedure codes are 97802 and 97803)

  • Are there any restricted diagnoses? (ask specifically about the diagnosis code Z71.3 and if it’s covered as preventive)

  • Do I need a referral or prior authorization?

  • Is nutrition counseling covered when provided via telehealth?

  • How many visits are covered?

  • Do I have a deductible to meet before insurance pays?

  • Will I have a copay or coinsurance?

  • Is Mind Body Nutrition Counseling (NPI 1841838257) covered under my plan? If not, what are my out-of-network nutrition benefits?

  • Write down the date and reference number for your call.

 

 

Cigna and Blue Cross Blue Shield

I am in-network with this insurance.

I ALWAYS recommend calling your Insurance to check benefits for nutrition counseling. That said, most plans DO cover visits with me.

A doctor’s referral is typically not required, although helpful to have if insurance denies the claim.

UnitedHealthcare

I am out-of-network with this insurance.

UnitedHealthcare has very specific coverage requirements. Please read this thoroughly and contact me if you have any questions.

  • A medical diagnosis is required for coverage.

    • Since dietitians are not able to diagnose medical conditions, you’ll need to provide me with documentation of a specific

      ICD-10 code(s) from your doctor that I can use for billing.

    • You can…

      • Have your doctors office fax just your ICD-10 diagnosis list or a referral that includes the code(s). My fax number is 833.666.5394.

      • You may be able to login to your doctor’s patient portal and access the codes yourself. If so, take a screenshot and upload it into the patient portal or print it out and bring it with you to your appointment. Be sure whatever you give me clearly shows both your name and the code(s).

      • Diagnosis codes are often printed on the Explanation of Benefits (EOB) statements provided to you by your insurance company following your appointments. If so, you can upload an EOB from a doctor’s visit into the patient portal.

    • If I don’t have codes to bill with, you will need to self-pay.
