Many health insurance plans cover nutrition and counseling services. Telehealth is also covered by the majority of plans we accept. Each plan is very different. Please read the coverage details below. Coverage for nutrition services and counseling

Counseling Services:

In-network: Cigna, Aetna.

Out-of-network: If you do not see your insurance company, I provide a superbill upon request for you to submit for out-of-network coverage. Many insurances cover OON benefits. I am in the process of credentialing with additional carriers so please check back.

Nutrition Services:

In-network: Cigna, BCBS/Anthem, Aetna

Out-of-network: If you do not see your insurance company, I provide a superbill upon request for you to submit for out-of-network coverage. Many insurances cover OON benefits. I am in the process of credentialing with additional carriers so please check back.

If you do not see your insurance plan listed above you have the option of seeking out-of-network reimbursement.  Some insurance plans offer out-of-network benefits which means you can submit statements to your insurance for potential reimbursement.  If you have questions about your out-of-network benefits, please contact your insurance company directly.  Contact information regarding benefits can be found on your insurance card, usually listed as a phone number for “member services” or “customer support.”

Coverage varies greatly between plans. Please scroll down for help determining what your plan covers. If insurance declines to cover your visit(s) for any reason, you are responsible for the full amount billed.

If we are out-of-network with your insurance, sessions may still be covered. Upon request, we can either send a claim to your insurance on your behalf or provide you with a superbill to submit yourself for possible reimbursement. MNBC can not submit superbills without a medical diagnosis. Payment is due from you at the time of service.

Payment:

We accept cash, check, credit/debit cards, HSA & FSA cards.
A credit/debit card on file is required to secure appointments.

BEFORE YOUR VISIT, PLEASE CALL YOUR INSURANCE COMPANY AND ASK:

  • Is nutrition counseling covered? (you can find the billing codes to give to insurance in our intake paperwork we will send you after scheduling your initial appointment)
  • Are there any restricted diagnoses? (ask specifically about the diagnosis code Z71.3 and if its covered as preventive)
  • Do I need a referral or prior authorization?
  • Is nutrition counseling covered when provided via telehealth?
  • How many visits are covered?
  • Do I have a deductible to meet before insurance pays?
  • Will I have a copay or coinsurance?
  • Is Mind + Body Nutrition Counseling covered under my plan? If not, what are my out-of network nutrition benefits?
  • Write down the date and reference number for your call.

Doing this will help ensure you get the most out of your benefits and avoid financial surprises!

INFORMATION ABOUT YOUR INSURANCE COMPANY

We have listed some information below regarding insurance coverage for nutrition therapy based upon our knowledge and/or experience to date with the companies.

Please understand that we absolutely cannot know the details of each plan and that the information below is not a guarantee of coverage. We do not regularly check benefits for clients so it is your responsibility to know what your plan covers and what your portion of the cost is. Mind + Body Nutrition Counseling will automatically resubmit denied claims on your behalf.

All clients will get an Explanation of Benefits (EOB) from their insurance company. You may or may not receive the EOB before MBNC receives the EOB and runs the charges. Clients will receive a receipt that will indicate if the charge is a copay, deductible, or coinsurance with the date of service the charges are for. Clients will not receive a bill prior to every card we will run payments for. Clients are responsible for understanding their benefits, checking, and keeping track of their EOBs from their insurance company and we recommend you do this to avoid any financial surprises. EOBs will either be mailed or uploaded to the client’s healthcare portal. Our biller will contact you only if there is a problem running your card on file or if we are notified through your EOB that you have exhausted your nutrition benefits through your healthcare plan. If choosing to use insurance benefits, it is the client’s responsibility to understand their plan and its deductible. To find out that there is a deductible as part of the client’s plan after submitting the claim per the client’s request, Mind + Body Nutrition Counseling will have to charge that full standard rate billed to insurance. These rates will run higher than our private pay rates. By choosing to use insurance, you will be responsible for this full payment. If clients have a high deductible, clients may opt to do private pay instead.

AETNA

We are out of network with this insurance. Upon request, we can either send a claim to your insurance on your behalf or provide you with a superbill to submit yourself for possible reimbursement. We typically see Aetna reimbursement in this way.

CIGNA

We are in network with this insurance for both nutrition and behavioral counseling services.

While we ALWAYS recommend calling your Insurance to check benefits for nutrition counseling. That said, most plans DO cover visits with us.

A doctor’s referral is typically not required.

AETNA

We are in network for behavioral counseling services and out of network for nutrition services.

Upon request, we can either send a claim to your insurance on your behalf or provide you with a superbill to submit yourself for possible reimbursement. We typically see Aetna reimbursement in this way.

UNITEDHEALTHCARE/UMR

We are out of network with this insurance for both nutrition and behavioral counseling services.

 Upon request, we can either send a claim to your insurance on your behalf or provide you with a superbill to submit yourself for possible reimbursement. We typically see UnitedHealthcare reimbursement in this way.

BLUE CROSS BLUE SHIELD

We are in network for nutrition services and out of network for behavioral counseling services.

MEDICARE/MEDICAID

We are in network for behavioral counseling services.

Medicare currently only covers diabetes and renal diagnoses for nutrition services and dietitians cannot currently accept Medicaid in the state of Colorado. If you have Medicare/Medicaid and would like nutrition counseling, please inquire about sliding scale rates.

City of Fort Collins Employees

City of Fort Collins will reimburse $500/year per insured. They pay you directly by when you submit a form for reimbursement. You are responsible for payment at time of service, and I will provide the superbill and form to you.
